Responsibilities

• Progress Tracking: Monitor and document each member’s progress with follow-up.
• Data Collection & Reporting: Collect, analyze, and summarize treatment results and compile statistics for leadership.
• Collaboration: Work with community partners, healthcare providers, and social service organizations to ensure comprehensive care.
• Member Empowerment: Educate members on available resources and guide them through processes.
