Qualifications

• Experience in case management, social work, or healthcare advocacy.
• Strong communication and interpersonal skills with the ability to build trust with diverse community members.
• Excellent organizational and time management abilities to handle multiple cases effectively.
• Ability to collect, analyze, and report on data.
• Knowledge of community-based services and social programs.
• Commitment to the values LOVE, LEARN, GROW and supporting vulnerable populations.

Responsibilities

• Conduct intake interviews to determine member needs and tailor support.
• Advocate for members by connecting them with essential services and resources.
• Monitor and document progress with consistent follow-up.
• Collect, analyze, and summarize treatment results for leadership reports.
• Collaborate with community partners and providers to ensure comprehensive care.
• Educate members on resources and guide them through processes for empowerment.
