Job summary

CareNu is seeking a disciplined Member Engagement Coordinator to welcome Members and families, assess education needs, and connect them with the appropriate Care Manager. You will educate on ACO benefits and resources, while fostering trust and adherence through proactive engagement.

The role involves coordinating across departments, maintaining knowledge of community resources, and supporting ongoing quality improvement efforts in patient engagement.

Qualifications

• Bachelor’s degree in Human Services or related field preferred.
• Minimum of one year of healthcare related roles.
• Solid communication (verbal and written) skills.
• Highly organized and self-motivated.

Responsibilities

• Welcomes Members and families and informs them they are part of the network.
• Educates on benefits and resources available; explains Accountable Care Organization (ACO).
• Develops and maintains knowledge of high-need resources and community connections.
• Earns mutual trust with families and Members to build rapport and adherence.
• Performs ongoing engagement surveys and assists with smooth handoffs between departments.
