Job summary

Community Health Network is seeking a PRN Medication Reconciliation Technician to obtain and document medication histories in the EPIC EMR, following Joint Commission standards. The role involves interviewing patients and families, coordinating with outpatient pharmacies or care facilities by phone, and tracking interventions with the department's process.

High school diploma or GED is required. Licensure as a Pharmacy Technician by IPLA or as a Pharmacy Intern (IPLA) with future certification

Qualifications

  • High School diploma or GED required.
  • Pharmacy Technician license by IPLA or Pharmacy Intern license by IPLA with later national certification.
  • National Certification (PTCB or NHA) required within two years of hire or equivalence for interns.

Responsibilities

  • Interpret and document medication histories in EPIC prior to admission.
  • Conduct thorough interviews with patients and families.
  • Telephone interviews with outpatient pharmacies or long-term care facilities to obtain medication history.
  • Track interventions and productivity using the department's process.
  • Communicate with patients, families, and hospital staff professionally and regularly.

Shift

PRN (as needed)

Hours

24 hours per 6 weeks. Mix of days, evenings, and weekends. 8.5-hour shifts between 7:00am and 11:00pm.

The Medication Reconciliation Technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards.

Your exceptional skills

Interpret and enter “Prior to Admission” medication history into the EPIC electronic medical record

Perform thorough interviews with patients and family members

Conduct telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history

Track interventions and productivity using department’s process

Regularly and professionally communicate with patients, their families, and hospital staff

High School diploma or GED equivalent required.

Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA)

National Certification through the PTCB or NHA required within two years of hire

Or

Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Intern,you do not need to be nationally certified with PTCB or NHA
