PRN Medication Reconciliation Technician required to work 2 weekend shifts per month. Shift hours: 8 am – 4:30 pm.

Responsibilities

Interpret and enter “Prior to Admission” medication history into EPIC electronic medical record. Perform thorough interviews with patients and family members. Conduct telephone interviews with outpatient pharmacies, long‑term care facilities, or other locations for medication history. Track interventions and productivity using department processes. Communicate regularly and professionally with patients, families, and hospital staff.

Qualifications

• High School diploma or GED equivalent.
• Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA).
• Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as a Pharmacy Intern, no national certification with PTCB or NHA is required.
