We are looking for Pharmacist students for our paid Medication Reconciliation Technician positions. Flexible schedules for students. The Medication History Technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards.
Interpret and enter prior to admission medication history into the EPIC electronic medical record, perform thorough interviews with patients and family members, conduct telephone interviews with outpatient pharmacies, long‑term care facilities, or other locations that may provide medication history, track interventions and productivity using department’s process, and regularly and professionally communicate with patients, their families, and hospital staff.
High School diploma or GED equivalent required.
