Job summary

Rockingham County in New Hampshire is seeking a Medication Care Partner to deliver high-quality, person-centered care in a fast-paced setting. The MCP engages with residents and families, building authentic relationships to support overall wellness.

The role involves assisting with meals, laundry, showers, and vital signs, while observing changes and reporting to the Resident Care Coordinator. Must be CPR/BLS certified and MNA is required; NCCC welcomed but not required.

Qualifications

• High School Diploma or GED.
• Two years related experience in a Health Care setting and/or training.
• Equivalent combination of education, work and experience will be considered.
• Medication Nursing Assistant (MNA) required.
• CPR/BLS Certified.
• National Caregiver Certification Course (NCCC) welcomed, not required.

Responsibilities

• Deliver high quality, person-centered care to residents in a fast-paced environment.
• Initiate conversation and activities to engage residents and families.
• Provide hands-on care as needed, including meals, laundry, and showers.
• Monitor vital signs and observe changes; report to Resident Care Coordinator/Manager.
• Follow established safety and infection control policies and procedures.
• Observe and report unsafe situations or changes in resident condition.

SCOPE OF POSITION: Medication

Care Partners interact directly with residents and are an integral part of the resident experience. They deliver high quality, person-centered care in a fast-paced environment. By developing authentic relationships with residents and family members, they help residents lead lives of enrichment and ensure their wellness needs are met. We are committed to providing preventative, restorative, and person-centered care to all residents at Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. Our Medication Care Partners (MCP’s) are a vital part of our dynamic team! The MCP actively engages with residents in a friendly, caring, and warm manner by initiating conversation and activities while supporting residents in doing as much for themselves as possible and further, promoting positive leisure time. When needed, they provide hands‑on care that may include assisting with meals, laundry, showering, vital signs, and more. Our MCP’s are continually educated on the best documentation practices, and are also responsible for observing/reporting any changes or concerns of physical, psychological, and social changes to the Resident Care Coordinator or Resident Care Manager. Additionally, they are responsible for following all established safety and infection control policies/procedures and will also intervene/report unsafe situations.

• High School Diploma or GED; and
• Two years related experience in a Health Care setting and/or training; or
• An equivalent combination of education, work and experience will be considered.
• Medication Nursing Assistant (MNA) required.
• CPR/Basic Life Support (BLS) Certified.
• National Caregiver Certification Course (NCCC) welcomed, not required.

SHIFT DIFFERENTIAL:

• 2nd Shift: $2.75/Hour
• 3rd Shift: $3.50/Hour
• Weekend Premium: $2.00/Hour
