Job summary

Rockingham County, NH is seeking Medication Care Partners to deliver high-quality, person-centered care to residents in our facility. You will engage with residents and families, support wellness needs, and assist with aspects of daily living while following safety and infection-control policies.

Candidates should have a High School Diploma or GED, two years in a health-care setting, and MNA; CPR/BLS certification is required. Training opportunities and shift differentials are offered.

Qualifications

• High School Diploma or GED.
• Two years related experience in a Health Care setting and/or training.
• An equivalent combination of education, work and experience will be considered.
• Medication Nursing Assistant (MNA) required.
• CPR/BLS Certified.
• National Caregiver Certification Course welcomed, not required.

Responsibilities

• Interact with residents in a friendly, caring manner and engage in activities.
• Provide hands-on care including meals, laundry, showering as needed.
• Document changes and report to the Resident Care Coordinator/Manager.
• Follow safety and infection-control policies and procedures.

SCOPE OF POSITION

Medication Care Partners interact directly with residents and are an integral part of the resident experience. They deliver high quality, person-centered care in a fast-paced environment. By developing authentic relationships with residents and family members, they help residents lead lives of enrichment and ensure their wellness needs are met.

We are committed to providing preventative, restorative, and person-centered care to all residents at Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. Our Medication Care Partners (MCP’s) are a vital part of our dynamic team! The MCP actively engages with residents in a friendly, caring, and warm manner by initiating conversation and activities while supporting residents in doing as much for themselves as possible and further, promoting positive leisure time. When needed, they provide hands-on care that may include assisting with meals, laundry, showering, vital signs, and more. Our MCP’s are continually educated on the best documentation practices, and are also responsible for observing/reporting any changes or concerns of physical, psychological, and social changes to the Resident Care Coordinator or Resident Care Manager. Additionally, they are responsible for following all established safety and infection control policies/procedures and will also intervene/report unsafe situations.

• High School Diploma or GED; and
• Two years related experience in a Health Care setting and/or training; or
• An equivalent combination of education, work and experience will be considered.
• Medication Nursing Assistant (MNA) required.
• CPR/Basic Life Support (BLS) Certified.
• National Caregiver Certification Course (NCCC) welcomed, not required.

STATUS/SHIFT

Full Time / 2nd Shift

SHIFT DIFFERENTIAL

• 2nd Shift: $2.75/Hour
• 3rd Shift: $3.50/Hour
• Weekend Premium: $2.00/Hour
