Qualifications

• Experience with providers, pharmacies, or healthcare outreach programs is a plus.
• Proficiency in Excel is required.
• Strong communication and phone outreach skills.
• Ability to interact professionally with providers, pharmacy staff, and members.
• Strong organizational and documentation skills with attention to detail.
• Proficiency in Microsoft Excel and Microsoft Office Suite.
• Ability to track multiple outreach activities and follow up effectively.
• Knowledge of Medicare Advantage or medication adherence programs is preferred.

Responsibilities

• Conduct initial outreach calls to notify providers of open medication adherence gaps.
• Send reminder emails to providers and pharmacies regarding unresolved gaps.
• Perform follow-up outreach to ensure awareness and action on gaps.
• Contact pharmacies and members to confirm medications were picked up.
• Maintain logs and documentation of outreach attempts and communications.
• Track outreach activities in internal systems and spreadsheets.
• Utilize Excel to track activities, status, and reporting metrics.
• Collaborate with internal teams to support adherence programs and quality improvements.
• Ensure outreach and documentation comply with Medicare Advantage requirements and policies.
