Job summary

HealthCare Partners in Garden City, NY seeks a Quality Interventions Coordinator to connect with members, providers, and pharmacies for quality improvement. The role focuses on medication adherence, gap closure, and collaboration with clinical teams across Medicare, Medicaid and Commercial lines.

Responsibilities include high-volume outreach, resolving barriers, and supporting medical record retrieval and documentation in quality systems. Strong communication and collaboration are essential.

Qualifications

• Associate's degree is required or an equivalent combination of education and relevant experience.
• Experience with high-volume member outreach and quality improvement measures is preferred.
• Familiarity with Medicare Star Ratings, HEDIS/QARR requirements, and medication adherence measures is preferred.

Responsibilities

• Conduct daily outbound telephonic outreach to members at risk for non-adherence and to close care gaps.
• Identify barriers to adherence and coordinate with pharmacies and prescribers to resolve them.
• Build relationships with provider offices and pharmacies to support quality initiatives.
• Support record retrieval, data entry, and documentation in quality platforms.
• Maintain knowledge of HEDIS/QARR requirements and escalate issues to leadership.
