Job summary

ChenMed is seeking a Medication Adherence Technician to engage patients in discussions about adherence, coordinate with providers and pharmacy teams, and help patients overcome barriers to taking medications as prescribed. You will support enrollment, process prescriptions, and ensure timely delivery of adherence packaging.

In this role, you’ll contribute to a patient-centered experience, meet outreach targets, and collaborate with clinical staff to optimize patient outcomes across ChenMed

Qualifications

• Foundational knowledge of standard medication adherence practices and processes.
• Excellent written, verbal, listening and interpersonal communication skills.
• Strong organization, prioritization and attention to detail.
• Ability to travel up to 10% locally/regionally.

Responsibilities

• Makes outbound calls to patients and healthcare professionals regarding patient medications, adherence, and barrier removal.
• Works diligently to remove barriers to ensure prescription regimens are provided to patients.
• Calls the pharmacy or provider to assist patient with medication refills, medication synchronization, or 90-day supply requests.
• Facilitates and/or coordinates the process whereby prescriptions are sent into the pharmacy for patients, processed appropriately, adjudicated to third party insurance, and prepared for dispensing.
