Job summary

HealthCare Partners in Garden City, NY seeks a Quality Interventions Coordinator to serve as the primary telephonic contact for members, providers, and pharmacies. The role focuses on closing gaps in care and improving medication adherence across Medicare, Medicaid and Commercial lines.

The position requires an Associate's degree or equivalent, 2 years in healthcare, and strong communication skills. Base pay is $50,000–$60,000 with a 12.5% bonus potential and comprehensive benefits.

Qualifications

• Associate's degree or equivalent required.
• Certified pharmacy technician, medical assistant, or similar healthcare support credential preferred.
• Experience in managed care, health plan, or provider settings is desirable.

Responsibilities

• Conduct high-volume telephonic outreach to address medication adherence gaps.
• Identify and overcome barriers to adherence and support 90- and 100-day supply conversions.
• Build positive working relationships with provider offices and pharmacies to support outreach.
• Assist with medical record retrieval and data entry in quality platforms.
• Maintain accurate documentation of outreach activities and support quality initiatives.
