Job summary

Fourrivers is seeking a Value-Based Care Nurse Care Manager to support Medicare patients by identifying high-risk individuals, closing care gaps, coordinating care, and improving preventive and chronic disease management.

This role uses population health data and EHR information to guide outreach and follow-up, working with interdisciplinary teams to enhance quality and Medicare value-based care performance.

Qualifications

• Must hold an active Missouri RN or LPN license.
• Graduate of an accredited nursing program.
• Strong communication and patient engagement skills.
• Ability to work both independently and within a multidisciplinary team.

Responsibilities

• Identify high-risk Medicare patients and close care gaps.
• Coordinate care transitions and follow-up after hospital events.
• Outreach, education, and care planning to support preventive care and chronic disease management.
• Review data to identify risks and intervention opportunities.
• Collaborate with providers and teams to develop follow-up plans.
