Qualifications

• Minimum 1 year of RN experience required.
• Experience in utilization management, case management or transitions of care beneficial.
• Bilingual English/Spanish preferred.
• Ability to work in a hybrid schedule (clinic and remote).

Responsibilities

• Oversees chronic care and transitions of care management for high-risk patients.
• Acts as a resource for the multidisciplinary team on complex patient care.
• Performs triage via phone and routes issues for intervention.
• Ensures efficient patient transitions from acute to post-acute settings.
• Performs comprehensive assessments addressing physical, mental, and social factors.
• Coordinates care progression and communicates with the care team to optimize plan of care.
• Monitors readmissions and supports performance improvement initiatives.
• Obtains and manages patient medical records from various facilities.
