Job summary

Four Rivers Community Health Center is seeking an exempt RN responsible for developing and managing care for Primary Care Health Home patients. The role includes supervising team members, coordinating with primary and specialty care, and ensuring HIPAA compliance while applying evidence-based guidelines and data sources.

You will lead population health initiatives, assess social determinants of health, and use EHRs and related data portals to drive program effectiveness and care gap closures.

Qualifications

• Education: Degree enabling NCLEX eligibility to become an RN.
• Experience: minimum 3 years as an RN; care coordination preferred.
• Licensure: Missouri RN license required; BSN/MSN preferred.
• Familiarity with HIPAA and care management workflows.

Responsibilities

• Direct relationships with patients and coordination with care teams.
• Develop care plans and monitor medication adherence.
• Identify high utilizers and coordinate with inpatient and specialty services.
• Adhere to mission, vision, and values of Four Rivers Community Health Center.
• Utilize EHRs and data sources to evaluate population health programs.
• Lead and participate in quality improvement initiatives.
