Upward Health is seeking a Nurse Care Manager to coordinate care for high-risk patients through personalized care plans, in-home and telephonic outreach, and seamless transitions across the healthcare system. You will work with a multidisciplinary Care Team to optimize outcomes and address chronic conditions.

Requirements include an unrestricted RN license and a minimum of three years of clinical nursing experience, with preferred experience in care management or transitional care settings;

Formación

• RN license (unrestricted) required.
• Minimum of three years of clinical nursing experience.
• Care management or transitional care experience preferred; ICU/Med-Surg/ER experience valued.

Responsabilidades

• Coordinate care for high-risk patients with comprehensive plans.
• In-home and telephonic assessments and care planning.
• Medication reconciliation and ensuring continuity across providers and services.
• Collaborate with multidisciplinary Care Team to optimize outcomes.
• Manage multiple patient cases and document in EHR.
