Job summary

Intermountain Health is seeking a Nurse Care Manager to collaborate with physicians and a multidisciplinary team to improve outcomes for members with chronic conditions or complex needs.

The role focuses on assessment, planning, facilitation, navigation, coordination, evaluation, and advocacy to meet medical, behavioral, and social needs while promoting self-management and cost-effective care.

Qualifications

• Current license to practice as a Registered Nurse in the state of residence.
• Demonstrated experience in clinical nursing.
• Minimum 1-year experience in care management/navigation or closely related field.
• Strong written, verbal communication and conflict resolution skills.
• Intermediate computer software and hardware skills, ability to use multiple monitors.
• RNs must obtain BSN within three years of hire.

Responsibilities

• Develop, document and implement care plans addressing social, physical, mental, emotional, spiritual, and supportive needs.
• Educate patients and caregivers to help manage health care needs.
• Coordinate and facilitate communication among the member, family, care team, and other parties.
• Maintain records per NCQA, CMS and state EQRO standards.
• Enroll and follow members requiring longitudinal care management services.
• Ensure plans are evidence-based and aligned with payer contracts and care processes.
• Contact patients at prescribed intervals to assess progress and adjust goals.
