UnitedHealth Group is seeking a Patient Care Coordinator in Layton, UT. You will manage scheduling, patient demographics, and interactions with providers to support preventive care and Medicare-related activities.

The role emphasizes customer service, teamwork, and Lean processes, with typical hours Monday–Friday 7:30 AM–5:00 PM. Join a mission-driven team focused on improving health outcomes and equity.

Formación

• Minimum high school diploma or GED.
• At least 1 year of customer service experience.
• At least 6 months of experience in a healthcare office setting.
• Proficiency in Microsoft Office suite required.

Responsabilidades

• Provide exceptional service to all customers.
• Ensure demographics and insurance information are accurate in scheduling and practice management software.
• Schedule appointments and verify visit types and empanelment with PCPs.
• Collaborate with providers on Medicare patient care, including medications, HEDIS, and annual wellness exams.
• Coordinate with back-office staff to maintain smooth patient flow.
• Resolve service issues and escalate when needed.
• Assist team members and float to other areas as required.
