Job summary

Oak Street Health in York, PA is seeking a Patient Growth Specialist (Outreach Executive I) to connect Medicare-eligible seniors with our care model. This role is 60% field-based and 40% clinic-based, working to grow our local patient base through targeted community outreach.

You will generate high-quality leads, develop a strong sales pipeline, and coordinate with CRMs and Outreach leadership. A valid driver’s license and US work authorization are required, with weekend availability for events.

Qualifications

• 1+ years in Sales or Customer Service.
• Ability to cold call, manage digital lead channels, and influence stakeholders.
• Excellent verbal and written communication with a professional phone presence.
• Must have a valid driver’s license and reliable transportation.
• US work authorization.
• Weekend availability for community events.
• CRM and/or EMR software experience is a plus.
• Fluency in another language is a plus.

Responsibilities

• Generate high-quality leads by executing community events and conducting localized outreach within the Medicare senior community.
• Identify and convert leads through cold calling, digital channels, and referrals from the broader outreach team.
• Persistently engage prospective patients via email, phone, and face-to-face networking to build a robust sales pipeline.
• Stay current on service updates, marketing initiatives, and organizational workstreams through active participation in team briefings.
• Partner with Community Relationship Managers (CRMs) and Outreach leadership to align on lead generation strategies and IA partnerships.
• Meticulously document all customer interactions and sales activities within the CRM and internal business systems.
• Serve as the primary liaison between clinical teams and new patients to ensure a seamless transition into our care.
• Address and resolve prospect concerns or complaints with empathy, ensuring a positive first impression of Oak Street Health.
• Maintain consistent touchpoints with prospects to foster trust and long-term community presence.
