Job summary

Oak Street Health is seeking a Patient Growth Specialist to connect Medicare-eligible seniors with our network. You will drive local patient growth as a brand ambassador, using outreach events and multi-channel engagement to build trust in our care model.

Reporting to Outreach leadership, you will collaborate with CRMs and IA partnerships, while maintaining a robust sales pipeline and precise CRM documentation to ensure a seamless patient experience.

Qualifications

• 1+ years in Sales or Customer Service.
• Must have a valid driver's license and reliable transportation; 60% field-based.
• Able to cold call, manage digital lead channels, and influence stakeholders.
• Excellent verbal and written communication with a professional phone presence.
• US work authorization.

Responsibilities

• Generate high-quality leads through community events and localized outreach.
• Convert leads via cold calls, digital channels, and referrals.
• Engage prospects via email, phone, and in-person networking to build a sales pipeline.
• Document all interactions in CRM and align with team workstreams.
• Serve as liaison between clinical teams and new patients for a smooth transition.
