Job summary

Caring Hand Home Health Solutions, Inc. is seeking a Medical Social Worker Manager to lead the social services team in our California operations. This role includes mentoring staff, shaping home-based care, and ensuring high-quality service consistent with state standards.

The position is per diem with a rate of $100 - $120 per visit, with potential to convert to full-time based on performance. Strong leadership and regulatory knowledge are essential to drive patient-centered outcomes.

Qualifications

• MSW from an accredited program and an active California LCSW license.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication skills and ability to build trust with patients and staff.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care and documentation.
• Partner with the Executive Director on the department budget and resources.
• Build knowledge of community resources and connect patients to support.
• Plan in-service training and continuing education to maintain skills.
• Contribute to quality improvement initiatives and patient service enhancements.
• Perform other duties as required for patient services.

• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
