Job summary

Best Home Health Providers, Inc. is seeking a Medical Social Worker to lead our home health social services team in California. You will mentor staff, ensure quality care, and partner with clinical leaders to support patients transitioning home from facilities.

The role is per diem, with compensation of $100–$120 per visit, and offers opportunities to grow towards full-time based on performance. Strong clinical knowledge and leadership experience are essential.

Qualifications

• MSW from an accredited program.
• Active California LCSW license required.
• At least 3 years of management experience, preferably in home health care.
• Strong knowledge of healthcare regulations and ACHC standards.

Responsibilities

• Lead daily operations of the social services team and ensure care coordination.
• Recruit, onboard and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services with proper documentation.
• Partner with Executive Director on department budgeting and resource planning.
• Connect patients and referral sources with community resources.
• Plan in-service trainings and continuing education to maintain high skills.
• Contribute to quality improvement initiatives and patient care enhancement.
• Perform other duties as required for patient services.

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

Requirements

• A Master of Social Work (MSW) from an accredited program

• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
