Job summary

JCFS Chicago, located in Chicago, IL, is seeking a Medical Services Coordinator (MSC) to coordinate medical, psychiatric, and health-related services for individuals with IDD. The MSC will work under the RN Trainer, ensuring appointments, documentation, and follow-up align with agency policies and DHS rules.

Qualifications

• Must be at least 21 years old.
• High School Diploma or GED required; CNA preferred.
• Satisfactory STOFHLA literacy.
• Completion of DSP curriculum as specified by DHS.
• CPR and First Aid training.
• Ability to lift up to 50 lbs.
• Two years of experience working with adults with IDD.

Responsibilities

• Schedule and coordinate medical, psychiatric, dental, laboratory, PT, OT, SLP, and other health-related appointments.
• Monitor appointment frequencies, follow-up requirements, referrals, labs and due dates.
• Coordinate transportation, documentation, and communication with individuals, guardians/POAs, providers, and the support team.
• Provide medical, behavioral, and functional information to providers as directed.
• Facilitate appointments and advocate for the individual's needs and outcomes.
• Obtain provider documentation, prescriptions, orders, signatures, and summaries.
• Ensure follow-up appointments, referrals, labs, and recommendations are scheduled and tracked.
• Report urgent medical information to RN Trainer promptly.
• Coordinate psychiatric appointments and relay changes to RN Trainer.
