The University of Illinois Chicago seeks a Medical Social Consultant (Care Coordinator) to provide care coordination services for DSCC Home Care. You will build partnerships with families enrolled in the program, conduct monthly interactions, and develop comprehensive, person-centered care plans for Medicaid/non-waiver and MFTD waiver participants.

Responsibilities include health assessments, documentation within 48 hours, interagency collaboration, and 6-month in-person visits.

Qualificações

• Master's degree in social or behavioral science or a related health field.
• Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker.
• Three (3) years of progressively responsible social work experience in a medical/clinical or social service setting.
• Illinois Residency is required within 180 days of employment.

Responsabilidades

• Provide active care coordination with comprehensive health assessments and a person-centered service plan.
• Ensure understanding of waiver services, estimated frequency, and provider type by participant or guardian.
• Facilitate 30-day monitoring of the care plan and identify unmet needs.
• Promote collaboration with HFS, DCFS and other agencies for participant care.
• Educate and connect families with resources for age transitions and health improvements.
• Complete documentation within 48 hours to meet waiver renewal requirements.
• Conduct in-person visits every 6 months or as needed in homes, schools, or hospitals.

Formação académica
