Job summary

University of Illinois Chicago (DSCC Core/Connect Care) seeks a Medical Social Associate to provide care coordination to families eligible under DSCC and LCDA programs. You will engage families, complete comprehensive assessments, develop person-centered care plans, and coordinate resources under supervision.

The role emphasizes documentation, collaboration with Medicaid teams, verification of resources, and supporting transitions with a culturally competent approach.

Qualifications

• Bachelor's degree in the social or behavioral science or related health specialty from an accredited college or university.
• Master's degree in social or behavioral science or related health specialty area from an accredited college or university.
• One (1) year (12 months) of progressively more responsible full-time experience in the practice of social services/ agencies or medical/clinical setting maintaining acceptable standards of social work practices.

Responsibilities

• Under the direction of the regional manager, performs active care coordination services by completing comprehensive health assessments, identifying families' strengths, and developing a person-centered care plan.
• Join and participate in Medicaid managed care clinical rounds, or other interdisciplinary care team meetings with participants’ providers, family members, or school teams.
• Utilize interpreter language line and accommodation resources based on ADA guidelines; comply with policies; participate in post-records reviews and remediation timelines.
• Assist families with private/public health insurance through effective benefits management; identify financial needs and assist with DSCC applications.
• The list of responsibilities is not all-inclusive and could be extended to include other obligations or tasks as indicated.
