Job summary

Ultipro in Chicago, IL is seeking a Health Services Coordinator (RN) to support community living arrangements for individuals with developmental disabilities, assessing, evaluating, and coordinating medical, health, and safety needs.

The role reports to the Director of Nursing Services. Requires a current Illinois RN license and two years of direct nursing with developmentally disabled adults; travel up to 50% and on-site, full-time schedule.

Qualifications

• Must hold current Illinois RN license.
• Two years direct nursing with developmentally disabled adults.
• CDDN preferred; RNT certification required within 12 months.

Responsibilities

• Provide health evaluations and monitor medical, health, and safety needs.
• Oversee Direct Support staff for medical needs and medication administration.
• Coordinate with PCPs, labs, pharmacies and other providers.
• Participate in admissions and discharge processes and hospital follow-ups.
• Train staff in medication administration.
