Job summary

Qualifications

• Master's degree in social or behavioral science or related health field.
• Current IL licensure as LSW or LCSW.
• Or minimum 3 years of progressively responsible social work experience in medical/clinical or social service setting.

Responsibilities

• Provide care coordination with comprehensive assessments and person-centered care plans.
• Engage families and partner with DSCC and other agencies to support care.
• Document within 48 hours to meet waiver and renewal requirements.
• Conduct in-person visits at home, school, or hospital every 6 months or as needed.
• Lead and participate in interdisciplinary care team meetings and resource referrals.
