Job summary

Illumination Health + Home is seeking a Medical Coordinator (LVN) to deliver high-quality medical care to patients experiencing homelessness in recuperative care settings. Flexible schedules include weekends and nights for per-diem positions.

The LVN must be licensed in California and have approximately two years of relevant nursing experience, with strong skills in patient assessment, care planning, and coordination of services.

Qualifications

• Current and non-restricted LVN license in California.
• 2 years of relevant nursing experience.
• High School Diploma or equivalent.

Responsibilities

• Perform intake to establish an appropriate care plan.
• Assess patient health and vitals on arrival.
• Maintain and update care plans and progress notes.
• Coordinate home health visits and follow-ups.
• Educate patients on illnesses, medications, and preventive care.
• Arrange transportation for medical appointments.
• Report emergencies and escalate when needed.
• Maintain med room and inventory.
