Job summary

Cleveland Area Hospital/Lake Area Medical Associates is seeking a detail-oriented Release of Information Clerk to support the medical records team. You will handle requests from physicians, patients, and agencies, ensure proper authorization, and log activity in the EMR or Excel.

Responsibilities include verifying records, tracking release processes, complying with HIPAA, and maintaining confidentiality while meeting deadlines in a clean, professional office environment.

Qualifications

• High School Diploma required.
• Excellent organizational skills and ability to change priorities quickly.
• Experience with medical records is preferred.
• Ability to comply with HIPAA and Compliance Policies.

Responsibilities

• Adhere to organization policies, including HIPAA, confidentiality, and patient rights.
• Track and monitor the release of information requests in EMR or Excel.
• Review requests for completeness and legitimacy before release.
• Ensure proper authorization to release information is in place.
• Enter and log requests in the release of information software or EMR.
• Provide monthly reports on outstanding chart deficiencies.
• Answer phones and perform other duties as assigned.
