Qualifications

The Care Facilitator is one of a patient's first contacts with our ChenMed family and serves as a patient advocate delivering exceptional customer service. The role includes checking patients in and out of the clinic and providing basic secretarial and administrative support. A successful Care Facilitator is committed to providing superior service and views every interaction as an opportunity to build trust with patients.

ESSENTIAL JOB DUTIES / RESPONSIBILITIES :
  1. Interacts appropriately with patients, records information, greets, checks-in, checks-out, or schedules appointments. Conducts lobby checks to ensure organization and cleanliness.
  2. Serves as the gatekeeper for PCP and Specialist schedules, ensuring top patients are scheduled every 13 days.
  3. Uses computer systems to add new patient info, print forms, schedule appointments, respond to emails, and generate New Patient packets. Ensures contact info is accurate.
  4. Makes love calls, mails birthday and greeting cards to patients for special occasions.
  5. Answers phones, takes accurate messages, and directs messages appropriately.
  6. Communicates information about ChenMed clinical staff to patients and others.
  7. Verifies insurance, collects copays, and assists with scheduling follow-ups and referrals.
  8. Escorts patients to exam rooms, ensuring their comfort and wellbeing.
  9. Acts as HEDIS champion, providing visibility on HEDIS needs and reporting hospitalizations and census data.
  10. Presents customer service surveys at check-out and escalates issues for immediate resolution.
  11. Participates in daily and weekly huddles to discuss patient details.
  12. Manages transportation aspects, including driver lists, medication and grocery deliveries, Uber/Lyft rides, and behavioral challenges.
  13. Maintains confidentiality of patient information and medical records.
  14. Reviews patient info for completeness and accuracy.
  15. Transmits correspondence via mail, email, or fax.
  16. Performs other duties as assigned by management.
KNOWLEDGE, SKILLS AND ABILITIES :
  • Exceptional customer service skills and a passion for serving others.
  • Working knowledge of medical insurance and electronic medical records (EMR) systems.
  • Knowledge of ChenMed products, services, policies, and procedures.
  • Proficient in operating phones, computers, and basic IT systems.
  • Professional and courteous communication skills.
  • Attention to detail for accurate reports and data.
  • Outstanding verbal and written communication skills.
  • Strong listening skills.
  • Positive, professional attitude.
  • Proficiency in Microsoft Office Suite (Excel, Word, Outlook).
  • Willingness to travel up to 10% regionally.
  • Fluent in English, both spoken and written.
EDUCATION AND EXPERIENCE CRITERIA :
  • High school diploma or equivalent required.
  • Graduation from a recognized Medical Assistant program preferred.
  • At least 1 year of experience in a medical clinic or similar environment required.
  • BLS certification preferred.
