Job summary

The Center for Elders Independence, a California PACE provider, seeks a Primary Care Physician to lead medical care for enrolled participants. Under the Chief Medical Officer, you will guide the interdisciplinary team and ensure coordinated, high-quality services for elderly participants in our clinic settings.

Responsibilities include comprehensive assessments, care-plan development, and supervision of CEI nurses and PAs, with emphasis on family collaboration and effective care coordination.

Qualifications

• MD with active California license.
• Minimum 1 year experience with frail/elderly population.
• Board certification in Internal Medicine or Family Practice preferred.
• Geriatrics experience preferred.
• DEA certificate required.
• Basic Life Support certification required.

Responsibilities

• Provide comprehensive medical care guided by the PACE benefit.
• Conduct comprehensive assessments at defined intervals.
• Participate with the IDT in care plan development and implementation.
• Review and sign care plans every 6 months.
• Oversee CEI nurse practitioners and physician assistants.
• Coordinate follow-up and referrals with families and care network.
