Job summary

WholePerson Care Clinic (WPCC) is seeking a Medical Coordinator Assistant to support client health monitoring and recovery in Recuperative Care programs. You will assist with medication self-administration, coordinate follow-up care, and ensure accurate documentation in the EHR under supervision of the LVN and clinical team.

This on-site role emphasizes trauma-informed care, collaboration with housing and behavioral health teams, and adherence to HIPAA/OSHA policies.

Qualifications

• High School Diploma or GED required.
• CMA, RMA, or EMT certification required.
• Minimum 1 year of experience in a clinical, shelter, or social services setting; preferred.
• Experience with homelessness, chronic illness, or behavioral health challenges preferred.
• Familiarity with EHR systems and multidisciplinary care support.

Responsibilities

• Conduct daily well-being checks and clinical intake under supervision.
• Assist with scheduling and coordinating medical and behavioral health appointments, diagnostics, and follow-up visits.
• Support clients in medication self-administration and monitor adherence to care plans.
• Document health-related interactions, medication logs, and care coordination in the EHR; ensure accuracy and timeliness.
• Collaborate with case managers, behavioral health staff, and housing teams to support holistic client recovery.
