Job summary

Whole Person Care Clinic (WPCC) is seeking a Medical Coordinator Assistant to support health monitoring, medication self-administration, and care coordination for Recuperative Care clients. You will work under the Medical Coordinator and clinical team in a trauma-informed, on-site setting.

Responsibilities include daily wellbeing checks, scheduling medical and behavioral health appointments, documenting in the EHR, and promoting adherence and preventive care within a multidisciplinary team.

Qualifications

• High School Diploma or GED required.
• CMA, RMA, or EMT certification required.
• Minimum 1 year of experience in a clinical, shelter, or social services setting.
• Experience working with individuals experiencing homelessness, chronic illness, or behavioral health challenges preferred.
• Familiarity with EHR systems and multidisciplinary care support.

Responsibilities

• Conduct daily well-being checks and support medical intake processes under clinical supervision.
• Assist with scheduling and coordinating medical and behavioral health appointments and follow-up visits.
• Support clients in medication self-administration and monitor adherence to care plans.
• Document all health-related interactions and care coordination in the EHR accurately and timely.
• Provide basic health education to clients focusing on adherence, hygiene, and preventive care.
