Job summary

WellLife Network seeks empathetic care professionals to accompany persons served to medical appointments and advocate for needed services. You will provide direct care, assist with ADL tasks, and help develop independent living skills while coordinating with nurses, physicians, and labs.

Qualifications

• High School diploma or equivalent required; Associate’s degree in Human Services preferred.

Responsibilities

• Accompany persons served to medical appointments and advocate for needed medical services.
• Provide direct care and train in activities of daily living and self-help.
• Safely operate agency vans per driving policy.
• Document medical information and care in the EHR system; coordinate with physicians, labs, and other providers.
• Maintain confidentiality at all times.
• Respond to potentially dangerous situations and provide emergency first aid as needed.
• Participate in weekly consult review meetings with Residence Manager and Residence Nurse.
