Job summary

Endeavor Health seeks a Medical Assistant in Warrenville, IL to join a CCM-focused program. The role supports complex patient needs, coordinates care, and maintains communication among patients, families, providers, and health plans.

Eligibility includes national MA certification within 3 months, BLS/CPR, and experience with clinical equipment. Full-time hours are Monday to Friday, 8:30 am to 5:00 pm with benefits available.

Qualifications

• Education: Preferred current certification from a national Medical Assistant certification body or obtain within 3 months of hire or transfer.
• Certification: BLS/CPR Certification for the Healthcare Provider American Heart Association (AHA) required
• Experience: Working knowledge of and experience with the use clinical equipment and materials.

Responsibilities

• Proactively identify CCM patients and support care coordination with providers and families.
• Support rising risk patients; assist MSW or RN with high risk patients.
• Engage and enroll patients in CCM and manage portal tasks for outreach.
• Outreach CCM patients; minimum 20 minutes per month.
• Refer clinical issues to RN and connect patients to resources.
• Assist with appointment scheduling and coordination of care.
