Endeavor Health seeks a Certified Medical Assistant to join our Warrenville, IL outpatient team. This full-time role offers 40 hours per week, Monday through Friday, 8:00 am to 4:30 pm, with premium pay for eligible staff.

You will provide wellness coaching, patient education, and care management support within a multidisciplinary care team, coordinating with RN Care Manager and providers. CMA certification is required.

Competenze

• 2 years of experience in outpatient ambulatory care setting.
• 1 year of Epic user experience.
• Knowledge of insurance/payor mix.
• Experience with healthcare systems and community-based organizations.

Mansioni

• Provides support, empowerment, education, and care management to identified patients.
• Verifies eligibility based on criteria report.
• Identifies support needs for patients and caregivers.
• Develops/modifies care plans and assists patients with self-management.
• Assesses patients' activity, diet, smoking, and social/economic resources.
• Works with patients with readmission issues to prevent recurrence.
• Provides prescriptions, DME, and outpatient services as needed.
• Documents actions in EMR and maintains daily caseload.
• Coordinates with RN Care Manager to develop/modify care plans.
• Uses community resources to support patient care options.
• Requests referrals and facilitates access to community resources.
• Educates patients on evidence-based standards of care and self-management.
