Qualifications

Responsibilities

• Coordinate medical appointments and document communications for residents.
• Provide direct support to residents as needed.
• Review MARs and communicate with medical providers.
• Track injuries/falls and ensure medications are on site.
• Coordinate with CSRN and HealthCare Providers for follow-up.
• Attend weekly CSRN meetings and implement Life Plan.
• Assist residents with community resources and daily tasks.
