Job summary

SCO Family of Services in Jamaica, NY seeks a Medical Coordinator/Direct Support Professional to join our on-site team. In collaboration with the RN, you will schedule medical appointments, prepare forms, and ensure regulatory compliance while supporting individuals and staff.

The role includes coordinating staff coverage, accompanying individuals to appointments, maintaining medical binders, managing medications and pharmacy communications, and documenting activities in a timely, accurate

Qualifications

• Strong organizational and interpersonal skills.
• Good oral and written communication abilities.
• Valid NYS Driver's License is required.

Responsibilities

• In collaboration with RN, schedule medical appointments and ensure timely adherence to regulations.
• Prepare clinical consultation forms for all medical appointments.
• Ensure staff coverage for appointments and accompany individuals at visits when possible.
• Review and file medical documentation with RN/Residence Manager; maintain medical binders.
• Monitor prescription submissions, medication counts, and documentation in MAR records.
