Job summary

Family Residences and Essential Enterprises, Inc. (FREE) is seeking a Medical Coordinator OPWDD- Housing Services to coordinate medical and psychiatric services for adults with developmental disabilities in our Smithtown area residences.

This in-person role requires AMAP certification or the ability to obtain within 120 days, strong organization, communication, and teamwork to help residents achieve personal outcomes.

Qualifications

• High school diploma is required.
• Minimum of 1 year of experience providing services to individuals with disabilities.
• Valid AMAP certification required or must be obtained within first 120 days.
• Proficient computer skills and strong writing and communication abilities.

Responsibilities

• Coordinates and schedules medical and psychiatric services for adults supported.
• Maintains documentation of treatment and communicates with the care team.
• Maintains medication inventory and orders medications as needed.
• Sets up and manages filing systems and coordinates information flow.
• Ensures all required documentation is completed in a timely manner and compliant.
• Performs other duties as assigned.
• Travel between FREE locations and doctors' offices as needed.
