Job summary

SCO Family of Services seeks a Medical Coordinator/Direct Support Professional to schedule medical appointments, prepare forms, and support RN and Residence Manager in ensuring timely, compliant care. You will accompany individuals to appointments when possible and maintain accurate medical records.

The role requires strong communication, organization, and interpersonal skills, plus a valid NYS driver’s license. This is an on-site, full-time position based in New York.

Qualifications

• High school diploma or better required.
• Strong communication skills and reliability.
• Valid NYS Driver’s License is required.

Responsibilities

• Coordinate medical appointments with RN and Residence Manager.
• Prepare clinical forms and ensure documentation is signed.
• Assist with medication management and record keeping.
• Ensure medications are counted, stored, and ordered when needed.
• Maintain medical binders and AMAP documentation; ensure compliance with OPWDD regulations.
