The Center for Family Support Inc. seeks a Sr. DSP Medical Coordinator to handle duties of a Direct Support Professional plus scheduling, tracking and documenting medical appointments and labs. The role supports individuals with developmental disabilities in New York.

Responsibilities include maintaining medical logs, monitoring medication inventories weekly, and communicating with nurses and managers. A NYC driver license and relevant experience are required; a flexible schedule is beneficial.

Qualificações

• HS Diploma or GED required.
• 1+ year of HHA experience preferred.
• 2+ years of Direct Support Professional experience preferred.
• NYC Driver license required.

Responsabilidades

• Document all required paperwork, incident reports, and progress notes.
• Track medical appointments and follow-ups; document in tracking forms.
• Track lab tests and ensure proper documentation in medical logs.
• Manage medication inventory weekly; order as prescribed and maintain storage per regs.
• Identify medication errors and communicate to nurse/manager promptly.
• Use Medical Communication Log to update nurse and manager.
