Moms In Motion is seeking a Service Facilitator (SF) Case Manager to connect families with Virginia’s Medicaid Waiver programs and help them live safe, healthy, and independent lives. You’ll travel locally to client homes and coordinate plans of care with families and providers.

Role emphasizes compassion, organization, and strong communication. No degree is required; two years of relevant experience is preferred, and you’ll work remotely from a laptop/tablet while visiting clients across

Qualifikationen

• Minimum 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience valued.
• Valid driver’s license and reliable vehicle.
• Strong written and spoken communication.
• Ability to pass a background check and provide 2 professional references.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Bonus points for Person-Centered Thinking/Planning training.
