Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will visit families in their homes across Virginia, helping them navigate Medicaid Waiver programs and ensuring safe, healthy, independent living. The role emphasizes empathy, organization, and local travel, with a strong focus on client-centered outcomes.

The SF will write care plans, assist with authorizations and timesheets, and build lasting trust with families while working remotely from a laptop or tablet when not

Qualifikationen

• Experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience over formal education.
• Must be tech-savvy for email, docs, spreadsheets, portals, and video calls.
