Job summary

Norton Sound Health Corporation seeks a dedicated coordinator to support recipients under the Medicaid Home and Community Based Waiver program in Alaska. You will assist in completing waiver packets, liaise with medical staff and community providers, and help develop care plans that meet recipient needs.

You will travel to villages to meet recipients and families, collaborate with the Manager to prepare cost proposals, and participate in treatment teams to ensure integrated care and timely

Qualifications

• Knowledge of Medicaid waiver programs.
• Knowledge of Developmental Disability and related services.

Responsibilities

• Meet with eligible recipients, families and caregivers to assess needs.
• Complete and submit Medicaid Waiver packets and documentation.
• Contact medical professionals and community staff to obtain waiver information.
• Assist in developing Support Plans and service plans with the team.
• Collaborate with the program Manager on cost proposals and paperwork.
• Develop empathic relationships and refer to needed services.
• Participate in treatment team meetings and complete assessments.
