Norton Sound Health Corporation seeks a dedicated coordinator to support recipients under the Medicaid Home and Community Based Waiver program in Alaska. You will assist in completing waiver packets, liaise with medical staff and community providers, and help develop care plans that meet recipient needs.
You will travel to villages to meet recipients and families, collaborate with the Manager to prepare cost proposals, and participate in treatment teams to ensure integrated care and timely
The position coordinates available resources for recipients of Medicaid funded state Home and Community Based Waiver program services, through:
