Job summary
Western Reserve Area Agency on Aging is seeking a licensed Practical Nurse to serve as Waiver Service Coordinator. The role requires coordinating home- and community-based waiver services and working with an interdisciplinary team to support independent living for older adults and individuals with disabilities.
Travel throughout WRAAA's service area is required. A strong focus on care coordination, documentation, and collaboration with caregivers and providers is essential to ensure timely,
Qualifications
- Current and unrestricted Licensed Practical Nurse (LPN) license in Ohio.
- Graduate of an accredited Practical Nursing program.
- Experience with older adults and/or disabilities is preferred.
Responsibilities
- Coordinate and monitor authorized home- and community-based waiver services.
- Conduct member contacts and home visits as required.
- Assist with implementing and monitoring waiver service plans.
- Collaborate with Care Managers and the interdisciplinary team.
- Communicate changes in health or service needs to clinical staff.
- Assist members with accessing providers and community resources.
