Job summary

The Medicaid Waiver Case Manager position with South Alabama Regional Planning Commission will implement the Home and Community-based Services program within the SARPC area, adhering to Alabama Medicaid guidelines and coordinating with the Medicaid Waiver team.

As primary contact for referrals, you will conduct on-site assessments, authorize services, monitor care plans and billing, and perform annual redeterminations while maintaining accurate case documentation and client records.

Qualifications

• Knowledge of social work programs and processes.
• Knowledge of community resources available to clientele.
• Excellent oral and written communication skills.
• Ability to relate to the elderly and their problems.
• Knowledge and ability to read and interpret guidelines and regulations.
• Completion of initial case manager training within three (3) months of employment.
• A valid driver's license and a good driving record.

Responsibilities

• Establish referral sources and respond to referrals.
• Conduct on-site assessments to enroll clients in the Medicaid Waiver Program.
• Authorize contractors and forward care plans to contractors.
• Develop and monitor client care plans with monthly home visits.
• Check monthly billings for accuracy and conformity with care plans; monitor cost effectiveness.
• Conduct yearly redetermination assessments per guidelines.
• Prepare HCBS Case Management Activity Records and Daily Time Logs.
• Exchange information with Medicaid Waiver clerical staff to assign work and follow up on tasks.
• Other duties as assigned.
