NYREACH in the Bronx is seeking a Certified Application Counselor to coordinate Medicaid recertification and ensure continuous coverage for enrolled patients. You will guide six-month renewal cycles, handle enrollment through NYSOH, and support documentation for compliant reporting.

The role emphasizes bilingual Spanish/English communication, outreach to at-risk individuals, and collaboration with clinical partners to protect patient access to care.

Qualifikationen

• Associate’s degree required.
• NYSOH Assistor Certification obtained within first weeks; pass exam (80%+) before client contact; certification preferred.
• CAC, CPHQ, CCM or similar credentials considered.

Aufgaben

• Recertification & renewal assistance for Medicaid; guide renewal cycles to prevent gaps in coverage.
• Eligibility screening & outreach for at-risk patients; identify gaps and follow up.
• Complete and submit NYSOH enrollments and renewals with required documentation.
• Counsel patients on Medicaid vs Essential Plan vs QHPs; explain costs and credits.
• Document community engagement requirements (HR 1) and exemptions.
• Assist mixed-status/undocumented households with coverage options and public charge considerations.
• Schedule, remind, and minimize no-shows with clear follow-up.
• Provide referrals for appeals and LDSS as needed.
• Maintain accurate documentation in NYSOH and EHR while protecting PII.
