Elevance Health is seeking a CFSP Managed Care Coordinator for Forsyth County, NC. This field-based role travels to client sites, supports Medicaid-enrolled children and families, and collaborates with county welfare workers to meet health and social needs.

The position emphasizes trauma-informed care and integrated care management across multiple service areas. The role requires a BA/BS and at least 1 year of experience with the target population.

Formación

• BA/BS degree and a minimum of 1 year of experience working directly with people in the CFSP population or related community-based organizations.
• Two years of experience with child welfare is preferred.
• Strong collaboration, communication, and care coordination skills are required.

Responsabilidades

• Provide integrated whole-person care management under CFSP, coordinating across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet needs.
• Coordinate with County Child Welfare Workers to align health care with permanency goals.
• Work with a multidisciplinary care team to coordinate care for members.
• Conduct telephonic or face-to-face assessments to identify and manage member program needs.
• Identify members needing expanded services and coordinate with clinical teams.
• Set short- and long-term goals with members, caregivers, families, and physicians.
