Qualifications

• Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the CFSP population or related community-based organizations.

Responsibilities

• Provide integrated whole-person care management under CFSP, coordinating physical health, behavioral health, I/DD, LTSS and pharmacy.
• Collaborate with County Child Welfare Workers to align health care with permanency goals.
• Work with multidisciplinary care teams including primary care, behavioral health and specialists to coordinate care.
• Conduct telephonic or in-person assessments to identify and manage member program needs.
• Oversee non-clinical needs of members with chronic illnesses to optimize care utilization.
